CAPTURING STUDENT IMAGE CONSENT FORM

Throughout the course of the time of enrollment, photographs or videos of students may be taken for our school website, social media posting, marketing purposes, and media publications.
Please complete the form below, indicating whether you wish to include your child’s image in media such as newspapers, magazines, newsletters, social media, and other publications.

I understand this means my child/ward will not be photographed and will be omitted from photo or video opportunities. Their image will not be posted on school social media platforms or publications by the school regardless of the activity, events, excursions, or functions.

Acknowledging in the case of my child/ward participates in any and all events or school activities such as, but not limited to, school performances, competitions, charity events, excursions, and so on whether held on or off school grounds or open to the public that St. Luke’s Academy cannot guarantee that the capturing of the image of or postings will not occur by those in attendance. Additionally, I am aware parents/guests / professional photographers or members of the press attending any and all events may inadvertently post pictures with my child/ward without holding the school liable.

I also indemnify St Luke’s Montessori Ltd, Trading as St Luke’s Academy as well as all Directors, Trustees, Volunteers, and its employees against any actions regarding the use of the student’s photograph
