Permissions

I hereby give permission for my child/ward to attend swimming classes and or events either to be carpooled or driven by myself.
Please excuse my child/ward from participating in all swimming classes and or events. I agree to provide the required doctor's letter justifying the request.

I indeminify St. Luke's Montessori Ltd., Trading as St. Luke's Academy and or Casi Bambini as well as all Directors, Trustess, Volunteers and its employees against any actions regarding attending all swimming classes, events and or competitions and or carpooling or transporting to and from the repective venues.
