Checklist for New Applicants

Step 1 
• Application fee
• Application Form Completed (Online)
• Tour
• Medical Enrolment Form to be completed by the doctor most familiar with the student
• Copy of student’s ID card or Passport (Photo ID)
• Copy of student’s birth certificate
• Copy of both Parents / Legal Guardian’s photo ID cards and Passport bio-data pages
• Documentation of Legal Guardianship – if applicable
• School reports from the last 2 academic years for the applicant
• Any other assessments or developmental reports
• Student Referral Form
• Immunizations card
• Medical admission form. This form must be filled in by the long-standing physician-most familiar with the child’s medical history. (Online)
• Student Visa or Welcome Stamp visa for all Non-National Students (CARICOM/
National Students do not require a student visa)
* Note: Scanned copies and copies are acceptable but original documents will be
required on your visit to the school

Step 2
• Student Trial Day
• Parent Meeting with Co-Principal or Principal
• Finalisation of placement

Step3  – once an offered place has been accepted the following will be required to be completed.
• Emergency Medical Consent Form (Online)
• Adult Collection Form with student password (Online)
• Photo Consent form (Online)
• Food Omission Form (Online)
• Swimming consent form and waivers (primary students only 6yrs -11yrs) (Online)
• Uniform Form to Order: School uniform, PE shirt, and Swimsuit (Online)
• Relevant fees –Building Levy Fee and Tuition Fee
• Signed Enrolment Contract
• Complete the purchase of booklist items and all requirements to prepare for the start
