Throughout the course of the year, we have swimming classes and competitions scheduled for all Primary Department students. Please complete the form below.

PLEASE COMPLETE A SEPARATE FORM FOR EACH CHILD

Permissions

I hereby give permission for my child/ward to attend swimming classes and/or events, either to be carpooled or driven by myself.
Please excuse my child/ward from participating in all swimming classes and/or events. I agree to provide the required doctor's letter justifying the request.

I indemnify St. Luke's Montessori Ltd., Trading as St. Luke's Academy and/or Casi Bambini, as well as all Directors, Trustees, Volunteers and its employees against any actions regarding attending all swimming classes, events and/or competitions and/or carpooling or transporting to and from the respective venues.
